Il primo personaggio, di questa antichissima ed illustre famiglia, pare sia un Teodoro, vissuto nella prima metà del X secolo, che possedeva dei territori in Napoli, fra i quali un campo, detto Monticelli de li Caracciolo Battilocchi. Questi appare defunto in una pergamena del 976, pergamena che è il più antico documento autentico del cognome della famiglia. Il D'Engenio riporta un'antichissima iscrizione, rilevata in una piccola chiesa denominata S. Maria Assunta dei Caracciolo Battilocchi, dove il Teodoro fu sepolto assieme ad Uranie, sua moglie. Al tempo del ducato napoletano gli appartenenti alla famiglia figuravano tra i "nobiliores homines", che godevano i privilegi concessi, previo un pactum, dal duca Sergio.
