Der Überlieferung nach stammt sie von der spanischen Adelsfamilie Patricio ab, deren Ritter nach den aragonesischen Königen in das Königreich Neapel abgestiegen sein wollte und sich zunächst in Sizilien, in der Stadt Palermo, und dann in Kampanien, in der Stadt, niederließ Neapel. Die Mutation von Patricio zu Patriciello ist auf Phänomene der Dialektphonik zurückzuführen, die sehr häufig vor der Registrierungskristallisierung von Nachnamen auftreten.
