Capitani is widespread throughout the centronord, Capitanio has its highest concentration in the Veneto region of Veneto, but it also has stocks in Lazio, Abruzzi and Puglie, Captain would seem typically Sicilian from Palermo and Agrigento, with a stock also in Rome and small stocks in the Savona and Neapolitan areas. These surnames should derive from nicknames originated from the medieval word Capitaneus (captain, the highest authority of the town), nicknames rarely deriving from having been part of the family of a Capitaneus, more often to serve or in some way connected to a Capitaneo.
