Carfagna seems to have an important nucleus in Lazio, and stocks in the Piceno, Abruzzo, Molise, Neapolitan and Foggia areas, Carfagni is absolutely rare, Carfagnini is characteristic of the Abruzzi Molise area, Scanno and Sulmona in the Aquila area and Montorio in the Frentani in the Campobasso area, with a stock also in Rome, Carfagnino, almost unique, would seem to be from Lazio, Carfagno seems to be typical of Montella nell'avellinese, they could derive, directly or through hypocorisms, from the Latin name Carfanius, of which we have an example in a tombstone of the year 345: "Veneri Aug (ustae) sacr (um) / T (itus) Carfanius T (iti filius) Pol (lia tribu) Barbarus / Ti (berius) Claudius Spuri (filius) Papir (ia tribu) Timonianus / promagistri soc (iorum) IIII p (ublicorum) Afric (ae) dedicated (averunt) / Venustus vilicus aediculam ab only de his ref (ecit) ", or even from names of places like Carfagna in Molise, but it is also possible that they derive in some cases from nicknames linked to the dialect word abruz zese carfagna indicating a typical black wool of the mountains between Puglia and Abruzzo. We find traces of this surname in the second half of the 1200s with Petrus Carfagnus or de Carfagno feudatory Angevin in Irpinia, and with the Carfagna feudal lords of Poggio Sannita (IS) in the 16th century and in 1515 with a Salvitto Carfagna who bought from Bartolomeo Carafa the Castle de 'Pizzi di Palena (CH).
