Esta antiga e ilustre família originou-se na Lombardia e tomou o nome da Signoria de Castel Carcano, no ano 970. Acredita-se que um indivíduo desta família, defendendo Milão em 571 contra Lotário, rei dos húngaros, que tinha um cisne como uma crista de prata, com um golpe de machado tinha-lhe despedaçado a cabeça e o elmo, pelo que em memória deste facto os Carcani assumiram o cisne e o machado como suas armas. Dos milaneses, um ramo do Carcani passou para Bari em 1493 e depois para Trani em 1622. Gozava da nobreza nas cidades de Milão, Trani al Seggio di Portanova e Barletta. Monumentos desta família podem ser vistos em Bari no Duomo. Ele tinha o campo de Nicorvo, os marquesados de Anzano e Pugnago e o ducado de Montaldino.
