Selon Mugnos nous constatons que cette famille est d'origine purement italienne, dispersée dans diverses villes et principalement à Naples et en Sicile. Dans le premier, il dérive de celui de Pise, qui a obtenu la seigneurie de Gaeta du pape Grégoire II, d'où le patronyme, après l'avoir fait confirmer par Lothaire empereur d'Occident. Elle se vante des papes et des cardinaux ainsi que des personnages illustres. Les ducs de Laurenzana florissant à Naples, les princes de Caserta résidant à Rome, les marquis Sortino à Syracuse, les Gaetani de Naro et ceux de Lentini ont soutenu leur grandeur ascendante. Inveges nous fait penser que de ladite 1ère souche pisane trois passages furent effectués en Sicile ; d'abord avec Riccardo Gaetani sous le roi Guglielmo il malo ; selon 1370 avec Antonio Gaetani, un homme très riche de Catane ; troisième 1417 à Palerme avec Pietro Gaetani invité par le roi Alfonso.
