Stando al Mugnos troviamo che tal famiglia è di pura origine italiana, sparsa in varie città e precipuamente in Napoli ed in Sicilia. Nella prima deriva da quella di Pisa, la quale ottenne da papa Gregorio II la signoria di Gaeta, donde il cognome, avendone avuta la conferma da Lotario imperatore d'Occidente. Vanta de' pontefici e de' cardinali non che de' personaggi illustri. I duchi di Laurenzana fiorenti in Napoli, i principi di Caserta residenti in Roma, i marchesi Sortino in Siracusa, i Gaetani di Naro e quelli di Lentini ne sostennero l'avita grandezza. Fa riflettere Inveges che dal detto 1° ceppo pisano tre passaggi si fecero in Sicilia; primo con Riccardo Gaetani sotto re Guglielmo il malo; secondo 1370 con Antonio Gaetani, uomo ricchissimo in Catania; terzo 1417 in Palermo con Pietro Gaetani invitato da re Alfonso. fu questi maestro razionale della Regia G.
