Si vuole che sia originaria dalla Toscana o da Genova; passata prima in Napoli e poi in Sicilia, dove godette nobiltà in Messina nei secoli XIII e XIV e in Palermo. Un Pietro Sansalvo, figlio di Marco, ottenne il 22 novembre 1449 investitura della metà del feudo di Bertolino o Mezzo Catuso; del quale feudo vennero investiti il 30 ottobre ed il 21 gennaio 1517 Francesco; figlio di Pietro; il 29 aprile 1551 Calogero, figlio di Francesco, e il 30 gennaio 1563 Paolilla, figlia di Calogero. Un Egidio Sansalvo e Castiglia, di Messina, con privilegio dato in Madrid il 23 aprile 1573, esecutoriato in Palermo il 1 novembre dello stesso anno, ottenne per sè e suoi concessione del titolo di regio cavaliere e dello stemma gentilizio; un Vincenzo, da Termine Imerese, il 30 aprile 1798, ottenne un attestato di nobiltà dal Senato di Palermo, fu segretario della Legazione del Re
