Alfei is specific to the central belt, in particular of the Macerata area of Tolentino and of Belforte del Chienti, and of Rome, Alfeo has a small Sicilian lineage in Aragon in the Agrigento region and in Gela in the Nyssa area and one in Taranto and Taranto, should derive from the Latin name of Greek origin Alpheus of which we have an example: "... Sed iugulare virum cum iam mucar pararet, Currentes Mauri socium de cede tulerunt. Signifer Alpheus medios violentus in hostes Currit, et exacuens fortes in bella sodales. Ut reliquas prosternit aves regina volucrum, Sic sic hostiles terit insisting phalanges ... ", the Sicilian lineage is most probably derived from the fact of inhabiting the family near the river Alfeo that bathes Syracuse.
